The flag originates from the personal collection of Apollo 16 CDR John Young, as certified by his widow, Susy Young: "I hereby certify that the large size United Nations flag accompanying this letter was flown to the Moon aboard the Apollo 16 mission April 16–27, 1972. It has been a part of the John W. Young collection since returning from the mission."
Further includes a letter of provenance signed by Charlie Duke, who notes: "I certify that the lunar orbit flown large format United Nations flag accompanying this letter is from the John Young collection. I further certify that this flag did, indeed, fly with us in the CSM Casper into lunar orbit during Apollo 16. It is one of only 8 such large format flags that John flew as a personal memento and had listed on his official and approved PPK list."
